JOB OVERVIEW: The position requires current Speech-Language Pathology to evaluate, plan, implement and delegate a comprehensive plan of care. Provision of therapy in a comprehensive manner to meet the needs of the patient with a safe discharge plan. Re-assessment/re-evaluation of the patient/client functional abilities are also essential functions of this position. Excellent communication with all types of patients/clients. Ability to assess the impact of simple to complex therapeutic modalities and procedures. Assistance with the completion of comprehensive treatment programs, discharge planning and family/caregiver training is a requirement of the position. Employment of excellent communication with the Clinical Partner and other members of the health care team is a requirement of the position. An understanding of VMC/Rehab Services Policies and Procedures, state/federal regulations and guidelines allow for appropriate provisions and billing of services.
Program development, quality improvement, staff development, marketing activities, team leadership and delegation, CFY supervision, student and volunteer supervision skills are a component of this position.

Perform accurate, appropriate and timely with completion of patient/client evaluations with completion of documentation according to the standards, protocols and pathways as appropriate.
Perform evaluations that include patient/client functional status appropriate to the patient/client diagnosis, needs or condition, and incorporate pain level if identified.
Ability to collect, analyze and integrate the information in the evaluations from all sources of the health care team including sources such as but not limited to; patient/family interview, patient observation, RN, MD and other discipline specific assessments. Ability to integrate this information to set appropriate plan of care for therapy services.
Ability to elicit information from patients/client on the patient identified needs and priorities and incorporate this into the assessment and plan of care.
Ability to set specific goals or discharge criteria in the plan of care as determined at the time of the evaluation. Specific goals will assist each patient/client in restoring, improving or maintaining each patient/client optimal level of functional performance in self-care, self-responsibility, independence and quality of life.
Discharge Planning with the medical/caregiver team will be part of the
assessment as needed for successful recovery or refer the patient/client to ongoing services to appropriately meet the needs of the patient/client.
Able to assess patients in 4 evaluation and treatment areas after initial orientation and mentoring.
CARE OF PATIENTS
Adheres to the Patient Care and Rehab Services Policies/Procedures and Treatment Protocols in all clinical areas.
If dedicated to another service, incorporates specific clinic/unit Policies/Procedures and Treatment Protocols into discipline specific clinical practice.
Ability to perform and document re-evaluations or re-assessments with the revision of treatment plans and goals according to the standards and at regular intervals. This is dependent upon the patient/client significant change in condition or diagnosis, or per standards of care.
Ability to perform and document re-evaluations or re-assessments that include
the patient's/client's response to care, pain level management as identified by the patient and appropriate to the treatment plan.
Ability to perform and document re-assessment or re-evaluation - of the patient/client functional performance and completion of goals set at evaluation. Determine the need for further treatment, discharge or referral to appropriate services.
CONTINUUM OF CARE
Attends team conferences/floor rounds an provides appropriate patient care
information and altars treatment plan as needed by incorporating information obtained at the conferences into the treatment plan.
Ability to appropriately document discharge or continuation of care in a timely and accurate manner.
Acts as a resource for staff for discharge planning and community resources
within VMC system and outside agencies to meet patient needs.
Ability to evaluate optimal level of care setting for patient and/or discharge as
appropriate to home programs or community resources, home safety
Ability to provide appropriate information to subsequent providers related to the patient/client’s plan of care and services when a patient moves to different services.
ENVIRONMENT OF CARE
Interacts appropriately with equipment vendors and gathers information on new products, evaluations, supplies, tests and provides this information to Director/ Supervisor or Clinical Lead as appropriate for quality patient care.
Actively seeks new ways to work with patients via. new equipment or supplies, new treatment or evaluation methods, and is seen as a resource to other staff in the use of equipment and treatment techniques.
Maintains safe and efficient patient care areas at all times for the safety of
patients, visitors and staff.
HUMAN RESOURCES –
Provide direct supervision to Rehab Aide staff for assistance with patient/client treatment programs.
Delegate patients appropriately to Rehab Aide assistant staff with clear communication and guidance as needed for successful patient care outcomes.
Provide education and answers to the Rehab Aide questions and provides feedback and education for assistance with treatment programs.
Provide clear and concise information to Rehab Aides for assistance in equipment procurement, patient transport, CQI, and assistance in patient care to allow for safe and effective patient treatments.
Provide clear and concise information to Support Staff for accurate assistance in patient scheduling, registration and financial information.
Orient new staff, students and volunteers as appropriate after completion of orientation to service, team or clinical area.
Provide observation student supervision experiences after orientation completion.
Current or in process of a Washington State Speech Pathologist license.
Must have Acute Care Experience
Current American Heart Association BLS/CPR for health Care providers
2 years of current clinical practice in an area of expertise and demonstrated leadership ability in the clinical setting. Demonstrated leadership can include: patient evaluation and treatment, program evaluation and development, staff/student education and training.
