The Speech-Language Pathologist CF serves as a member of the clinical team. He/she provides diagnostic assessment and appropriate intervention within their scope of practice.
Perform evaluations in accordance with discipline standards to determine person’s appropriateness for therapy and medical necessity.
Write detailed evaluation and assessment reports demonstrating the person’s abilities and areas for treatment.
Develop initial Treatment Plan detailing the person’s individualized therapy goals and update periodically, as per regulations.
Provide treatment in accordance to the person’s treatment plan goals using high standards of care.
Maintain clinic documentation in the person’s chart including: evaluations, treatment plans, assessments, correspondence, discharge summaries etc. Completes in a timely and accurate manner.
Work collaboratively as part of the clinical team. Attend meetings and share ideas regarding treatment as appropriate.
Be familiar with the regulations, policies and procedures of the clinic. Adhere to all HIPAA related policies and procedures safeguarding the protected health Information (‘PHI”) of all patients.
Submit billing for service rendered promptly. Choose the most appropriate CPT and ICD-10 codes.
Meets with supervisor on a regular basis and as required by Medicaid regulation and documents appropriately
Minimum Education: Master’s degree accredited by the council on Academic Accreditation in Audiology & Speech-Language Pathology. CCC is a plus.
Minimum Experience: Some experience working with adults is a plus.
Knowledge/Skills/Abilities (KSAs): Good oral and written communication skills; computer literacy; ability to work well with others; ability to speak, read and write English proficiently; exceptional interpersonal skills to be able to interact effectively with the people we support and their families/caregivers; thorough knowledge of speech-language pathology evaluation practices and documentation; knowledge of CPT and ICD-10 coding.
Physical Requirements: Prolonged periods of sitting; must have visual acuity adequate to read correspondence, computer screen, forms, etc.; manual dexterity; speaking and hearing ability sufficient to communicate by phone or in person at normal volumes; ability to lift up to 50 lbs. and to push wheelchairs, walking; bending; reaching.
Working Conditions: Office and therapeutic setting; subject to exposure of blood borne pathogens, dust, odors and other hazardous material
